I. Patient Rights

Patients of General Mental Health Services LLC have the right to:

  • Be treated with dignity, respect, and without discrimination, in accordance with the Americans with Disabilities Act (ADA) and federal law.
  • Receive care that protects privacy, safety, and confidentiality under HIPAA.
  • Be informed of their diagnosis, treatment options, risks, and benefits in understandable language.
  • Participate in treatment decisions and refuse care when permitted by law.
  • Request and receive clear information about office policies, fees, and treatment plans.
  • Review and obtain copies of medical records as allowed by HIPAA.
  • File complaints or grievances without fear of retaliation.
  • Receive services in an environment free from abuse, neglect, harassment, or exploitation.
  • Request reasonable accommodations for disability or communication needs.
  • Expect professional conduct from all staff consistent with clinical standards.

II. Patient Responsibilities

Patients receiving care from General Mental Health Services LLC are expected to:

  • Provide accurate and complete information about their health, history, and medications.
  • Follow the agreed-upon treatment plan and communicate any concerns.
  • Attend appointments and cancel or reschedule with at least 24 hours' notice.
  • Treat providers, staff, and other patients with courtesy and respect.
  • Fulfill financial obligations, including co-pays, deductibles, and fees.
  • Refrain from recording or sharing any portion of therapy sessions without consent.
  • Inform the practice promptly of changes in insurance, address, phone, or health status.
  • Cooperate in maintaining a safe, respectful, and confidential environment.
